Why does the map display on my Nissan Qashqai say the SD card isn't inserted when it is?

I have a four year old Nissan Qashqai. Recently the map display says the card is not inserted even though it is. Sometimes the problem cures itself and other times the card needs to be taken out and re-inserted....

Smoky Nissan Qashqai

My Nissan Qashqai diesel (2007 model) jolts along at high speeds or high revs. There's also thick black smoke emitting from the exhaust. Any idea what the fault could be?
