Toyota Verso jumping out of gear

My Toyota Verso keeps jumping out of gear. The car is a manual diesel and it usually pops out of fifth gear. How can I fix this? Is it likely to be an expensive fault?

Likely to need a transmission strip and rebuild and no point in doing that without a new clutch and DMF, so you're looking at £1,500 - £2,000.
