Oh dear, that doesn't sound like a cheap fix.
On a side note, and just in case it produces a happier diagnosis, I took tge gearlever gaitor off earlier and went for a drive.
What i can see from the gear stick is that it has two arms/rods with ball and socket type fixings at the top. As you move through the gears these rods move up and down and side to side.
When fifth gear is selected, the lower of the two rods vibrate which causes the rattle. If you twist the rod at the ball and socket end it completely removes any noise.
It may very well be the case that what you have described would cause the rod to vibrate but I thought I'd mention it just in case.
