Why won't my Honda CR-V do any better than 28 miles to the gallon?

I drive a 2003 Honda CR-V with 48,000 miles on the clock. I cannot achieve more than 28 miles to the gallon. I have tried four tanks of Shell V Power and still no improvement. It's proving expensive.
